To be clear, this is my 1999 Tacoma. It's completely stock as far as I can tell. Tires are exact stock size as well. But my speedo is 10 mph off. And it's not gradual. It's off 10 mph at 40 and 50 and 60... I can't say I've noticed if it just fails to register from 0-10 or where it starts to go off. Guess I cold give it a drive later. I have a couple questions.

1. How do I fix it?

2. Does this mean that my odometer is showing that my truck has travelled more miles than it really has?

I always thought it was off because cars would blow by me when I thought I was going fast enough. Needed the GPS the other day though and it comfirmed my suspicions.

Depends on where the problem is, to figure out if your miles are accurate. It might be as simple as the speedo neatle is not on the shaft correctly. I would check this first, maybe by matching up your odo to a gps odo?

or just reset the trip computer at a mile marker...

You may want a more accurate reset. If you do the mile marker @ 75 mph, do it over a few miles. Smartphones all have free gps speed/odo apps.

Also- if the truck has any kind of after market gauge insert, that were highly popluar in the late 90's, I would guess someone put the neatles on poorly.

I'll steal the GPS back from my wife and take a 10 mile drive by the odeometer and see how close it comes out. If it's accurate then the speedo needle just needs adjusting?

WTF is a neatle? Does your internet not include a spellchecker?
