What happened was... (actually got this from nopistons.com)

The guy bought some new lug nuts, wrong size. They threaded on and tightened down (imagine a 1/2" nut on a 7/16" bolt) , but once some force was put on them they came off.

EDIT: but yeah this video is older than your grandma.

I kinda figured it had to be something along those lines. Thats the only logical explanation
