I have a friend who has a 2004 3/4 ton truck with a 6.0L that is not idling correctly. We have cleaned the maf and replaced the maf with the same results. What happens is it starts up rough then idles at 500rpm. Then when you stop at a light or stop sign it surges between 500rpm and 200rpm. Today it actually died at a light for the first time. Any help that I can get is greatly appreciated.

Leak in the air intake tract after the MAF. unplug the maf and run it, if it runs better then start looking for leaks. Also watch the trims ona scanner to see if one or both banks are going apeshit. If one bank is single handendly tanking out then there is an issue on that bank, either the 02 is dead or a decent exhaust leaks 6 " before or aft that sensor. If both are fairly close to eachother and tanking, then its still back to the maf issue. Surging and stalling and low idle = TOO MUCH FUEL on these motors.

We cleaned the tb and now it runs like a million bucks. Went through a whole bottle of carb and choke cleaning it. Thanks for your help though Tom.

hmmm thats and ETC truck too... must have been a fuck load of carbon to do that! Or the air leak was fixed in the process.

Yeah it is an ETC truck. It use to be a fleet truck and has about 160K on it. The tb had a lot of build up on it.
